What is the SAINT protocol?
Think of it as TMS on fast-forward. The Stanford Accelerated Intelligent Neuromodulation Therapy (SAINT) protocol compresses an entire TMS treatment course into 5 consecutive days. Where standard TMS gives you 36 sessions spread over 6-9 weeks, SAINT delivers 50 sessions — 10 per day — in one work week.
Developed at Stanford University, it’s the most aggressive TMS protocol available. And the results have turned heads.
How SAINT differs from standard TMS
| Factor | Standard TMS | SAINT Protocol |
|---|---|---|
| Total sessions | 36 | 50 |
| Daily sessions | 1 | 10 (with 50-min breaks) |
| Total duration | 6-9 weeks | 5 days |
| Targeting | Standard 5cm rule | fMRI-guided, patient-specific |
| Pulses per session | 600 (iTBS) or 3000 (10Hz) | 1,800 (modified iTBS) |
| Total pulses | 21,600-108,000 | 90,000 |
The three innovations
- Accelerated dosing: 10 theta burst sessions per day, spaced 50 minutes apart, tapping into the brain’s reconsolidation window for enhanced neuroplasticity
- Precision targeting: You get a functional MRI so clinicians can pinpoint the exact DLPFC subregion most strongly connected to the subgenual anterior cingulate cortex — a key depression hub in the brain
- Higher total dose: 90,000 total pulses over 5 days. That’s significantly more than standard courses deliver
Clinical evidence
The numbers from the landmark 2022 randomized controlled trial (published in the American Journal of Psychiatry) are striking:
- 79% remission rate vs 13% with sham. Read that again.
- 90% response rate in the open-label phase
- Most people improved within the 5-day treatment period itself
- Effects held at 4-week follow-up
- Well tolerated, no serious adverse events
These are some of the best outcomes in the entire TMS literature.
Who qualifies
SAINT is generally offered to people who:
- Have treatment-resistant major depressive disorder
- Have tried 2+ medications without enough relief
- Can commit to 5 consecutive full days (8+ hours per day at the clinic)
- Can undergo a brain MRI (no metal implants, no severe claustrophobia)
- Have no seizure disorder or other TMS contraindications
The treatment experience
Here’s what a SAINT week actually looks like:
- Day 0 (pre-treatment): Brain MRI for targeting, plus clinical assessment
- Days 1-5: You arrive early morning, then receive 10 iTBS sessions throughout the day with roughly 50-minute breaks between each one
- Each session lasts about 10 minutes
- Between sessions? Rest, read, eat, watch something. It’s a lot of waiting.
- Most people describe increasing fatigue as the week goes on, but the discomfort stays manageable
Where to find SAINT
SAINT is still rolling out, but availability is growing:
- Stanford University — where it all started
- Several academic medical centers offering “SAINT-like” accelerated protocols
- Specialized TMS clinics that have adopted the full protocol
- Not yet available at most standard TMS clinics
Cost and insurance
Let’s be direct about the money:
- Cost: $10,000-$25,000 (includes MRI, 50 sessions, clinical oversight)
- Insurance: Coverage is evolving. Some insurers cover it as “intensive TMS.” Others deny it as experimental.
- Prior authorization: Expect more paperwork than standard TMS requires
- Ask the providing center about their insurance track record — they’ll know which carriers have been cooperative
Limitations
- Five full consecutive days is a real schedule commitment
- Requires a brain MRI (additional cost if not covered)
- Not yet available at most TMS clinics
- Long-term durability data is still being collected
- Real-world results may not match the clinical trial numbers — patient selection and protocol adherence matter

How This Protocol Compares
| Protocol | Session Time | Total Course | Best For |
|---|---|---|---|
| Standard rTMS | 19-37 min | 6-9 weeks | Depression (most studied) |
| Theta Burst | 3-9 min | 6-9 weeks | Depression (time-efficient) |
| SAINT Protocol | Multiple/day | 5 days | Rapid response needed |
| Deep TMS | 20-30 min | 6 weeks | OCD, smoking cessation |
Finding a Provider
Not every TMS clinic offers every protocol. When searching, ask specifically which protocols they support and which devices they use. Our clinic directory helps you find experienced providers in your area. For protocol-specific questions, the treating psychiatrist should be able to explain why they recommend one approach over another for your situation.
