Blue Care Network (BCN), Michigan’s Blue Cross Blue Shield HMO, covers TMS therapy for treatment-resistant depression under their behavioral health benefits. BCN is part of the BCBS Michigan family and follows their TMS medical policy with some plan-specific requirements.

BCN Coverage Requirements

To get TMS approved through BCN, you’ll typically need:

1. Diagnosis: Major Depressive Disorder confirmed by a psychiatrist
2. Medication trials: At least 2 failed antidepressant trials at adequate dose and duration
3. Psychiatric evaluation: Current assessment documenting treatment-resistant status
4. PHQ-9 score: Documented depression severity
5. Prior authorization: Required before starting treatment

Authorization Process

1. Referral: Get a referral from your BCN primary care physician or psychiatrist
2. Pre-authorization: Your TMS clinic submits authorization to BCN
3. Documentation: Include medication history, PHQ-9 scores, and treatment plan
4. Response: BCN typically responds within 5-15 business days
5. Sessions: Authorization covers 30-36 TMS sessions

BCN members have access to TMS providers across Michigan:

• Southeast Michigan (Detroit metro): Henry Ford Health, Beaumont, and numerous private TMS clinics
• Grand Rapids: Spectrum Health TMS program
• Ann Arbor: University of Michigan Health System
• Lansing: Sparrow Health System
• Flint, Saginaw, Traverse City: Regional TMS availability

Cost Sharing

• In-network: Specialist copay or coinsurance after deductible
• BCN Medicare Advantage: $30-$50 specialist copay per visit
• BCN Commercial: Deductible + coinsurance, typically 20%
• Out-of-network: Higher cost sharing, may not be covered in full

BCN Medicare Advantage (Medicare Plus Blue)

If you have BCN’s Medicare Advantage plan (Medicare Plus Blue PPO or PMD), TMS is covered with prior authorization. Your out-of-pocket is limited to your plan’s annual maximum.
